Data subject

This consent is granted by a natural person (hereinafter — the Data Subject) to the personal data operator — Doctor.Nola LLP (hereinafter — the Operator), which carries out its activities within the territory of the Republic of Kazakhstan in accordance with the Law of the Republic of Kazakhstan "On Personal Data and Its Protection".

List of data

The Data Subject consents to the processing of the following personal data:

• Surname, first name, patronymic
• Date of birth, sex
• Mobile telephone number and email address
• Medical information: diagnoses, prescriptions, test results
• Insurance policy details (if any)

Purposes of processing

Personal data is processed solely for the purposes of:

• Maintaining the medical record and treatment history
• Appointment scheduling, referrals and routing
• Issuing invoices and interacting with insurance organizations
• Complying with the requirements of healthcare legislation

Term of validity

The consent is valid indefinitely from the moment it is granted and until the Operator receives a written withdrawal. After the consent ceases to be in effect, the data is retained for the period established by the legislation of the Republic of Kazakhstan for medical documentation — not less than 25 years.

Withdrawal of consent

The Data Subject has the right to withdraw consent at any time by sending a written application to the Operator's address or by contacting the registration desk of the medical institution in person. Withdrawal does not invalidate actions carried out prior to its receipt.
